It froze on (my brother). He loaded 4-11X into the level editor to make an expert remix of it and while playtesting as meat boy it 'froze' like this:
1) Reached bandage girl 2) Hit saw 3) Continued flying right until he hit a block 4) The timer stopped 5) No controls worked: meat boy couldn't be moved, exit didn't work, no menus could be opened, etc. but the lasers still animated and made sounds, meat boy still breathed, if you focused away meat boy did the (splatter 100x times a second because the game thinks you're falling into the floor and ejects you ever frame) thing

-Make an option to select a "low death run." it would take you to the world map and show all the levels as incomplete. When you beat them, they show up as cleared. And if you died, you would get one to the death counter. It'd make keeping track of levels you've beat in such runs easier. Sorry it's not the easiest thing to explain.
You could make it like Jumper 2, which kept track of how many times you've died in each level.

Where's my napkins, mom?: Complete "Puberty" without dying. (hard one  ) But any successful completion of a level is 'without dying', since if you die you go back to the start I realize you mean 'a replay with no deaths in it', but then you just restart every time you die, which is painstaking for no good reason

I think my USB dancepad is still floating around somewhere too so maybe i will try to find that.
