Been playing a lot of Bit.Trip Runner recently. It's on-rails and some of the stages are fairly long, one screw up and you start the entire stage over. I do like how the music is "created" as you play the stages based on how well you're dodging obstacles and if you are Hyper/Mega/Super/Ultra/Extra. And the menu music I cannot get out of my head.

Super Mario Bros 2. The japanese version. If you die (pretty easy) you restart the world. If you are beat on 5-4, you begin on 5-1. Imagine that on Super Meat Boy. I really really enjoyed I Wanna Be The Guy. The last boss took like as long as the rest of the game, but the satisfaction was higher than 106% and A+ing Super Meat Boy. I liked it so much I downloaded I Wanna Be The Fangame too. Great leveldesign too.
Flail and Jumper, both from the same maker were also quite good.
And for as far as you can call Cave Story/Doukutsu a hardcore platformer, I've been enjoying that one lately too. The Sacred Grounds really take some practice, before you can finish those.

How has no one said Super Mario Bros 3? True, SMB: The Lost Levels (aka SMB2 JPN) is harder, but SMB3 had pretty much everything going for it. Sweet 8-bit graphics, good soundtrack and it was just f***ing awesome. As for Mega Man,I would say 9 is the hardest but I think 3 is the best. Also, am I the only person on the planet to like Mega Man 6? It may not have been as hard as the others but it was still a good game. The best 2D platformer ever though has to be Super Meat Boy. And i'm not saying that just cuz i'm on the forums!
