Apologies in advance if these are already known - i've read the stickies and checked the wiki but didn't notice something similar.

Configuration (stuff bolded because I feel it may be important, but could be anything) :

Intel quad core (q6600), 4 GB DDR2, Radeon 4850 512MB of memory, onboard sound 5.1 (with 5.1 speakers) Windows 7 Home Premium 64 bit
Using 2 monitors, first 1920x1200 24", secondary 1920x1080 24"
Using keyboard (yes, i know...)

1. Occasional crash when loading when video runs on second screen.

If on the second display I have a video running in a player like Media Player Classic Home Cinema (which uses EVR renderer in case it matters) in a window, when I start the game, it feels like Super Meat Boy is trying to change the resolution (both screens black out and then back on) of the main display twice and then it crashes with a Windows error (the one with stopped responding, look for solution).
The second time I start SMB right after it crashed, it does that flicker only once and loads fine, just like it normally does.

I can't reproduce it all the time, i don't think it matters if the video player uses "DXVA" or whatever technology is called nowadays to decode video in video card's hardware because I've got this crash when I'm sure it didn't use hardware, as the video card can't decode regular xvid (mpeg 4 asp).

2. Freeze when replay starts

This one's simple, basically playing several maps trying to get bandages I missed before, usually pressing space to skip replay but after several maps it just freezes before replay even starts. Sometimes I manage 4-5 maps, sometimes it freezes even after the first map done.
I basically have to alt-tab and then alt-tab back and sometimes this is enough for SMB to say it generated a mini-dump and it sends it, sometimes it's the old Ctrl+Esc and kill process.

This is relatively a new thing, probably in the last 2-3 weeks it's much often.


3. Sound not managed properly

Setting music volume to something like 5%, anyway low. When SMB starts, the volume for the intros is high, so is the initial logo/whatever, up until "press start". The moment I press start and get the actual menu volume goes to the normal values.
This is annoying and I can live with it but some people may have this and get pissed because they want to play at night and wake up their girlfriend/etc.

4. Alt tab issues...

I guess this is already known... play a map, press Esc, press alt-tab do something for a bit of time, press alt tab or click SMB icon and crash/mini dump.
Lately I've seen it happen less often but as I type this I've just managed to crash the game by alt-tabbing after I press space to enter a warp zone (animation started, alt tab, it said mini-dump sent)
