That reminds me something. About what I think that the game could be.
So... Isaac's mother is extremly religious and think that his son is corrupted, so much that she have to sacrifice him. And that's what made Isaac become a monster (three eyes, red skin, etc).
So the end of the game is Isaac against his mother. I can imagine her talking to god and blaming his son to see how "corrupted" he become (due to his mother's problem) and that's her fate to kill him. Then, Isaac kill his mother. At this point, we can see the mom just a victim of some stuff from religion.
Also... secret ending if you dont pick any item = not "corrupted"?
Theres too much to dig.
