Well, I just bought SMB and in the CH2 Lvl 8 in the Warpzone, this happens to me.

Does it happen on other levels? Does it happen after you've restarted the game or the system?

Yeah, I've turn off my Xbox and started the game again, but I haven't uninstall it, maybe I should download it again.
