He once appeared in the COTTON ALLEY DARK WORLD STAGE SELECT SCENE

I was on level 1-5, retrying my Forest Boy runthrough. (On the previous run, I entered a warp by accident, so I restarted my run just to be sure). A flute or something similiar (I forget what) started playing on top of the regular music. I alt + escaped to see if I had a video, and the music stopped. So I went back into the game, and it crashed.  |

That's Meat Ninja making an appearance.
It doesn't do anything aside from scare the crap out of you because it happens at random, but it's a tip-of-the-hat to the character you unlock for getting 100%.
