Although the Salt Factory is my favorite, I voted for Cotton Alley as it presents a challenge.

Forest +: squirrels, easy -: boring visuals, too easy
Hospital +: lasers, purple, blood clots, lasers, relaxing color scheme, nice length of levels, Betus Blues, lasers -: problematic if you dislike purple
Salt Factory +: music, strong visual continuity, missile launchers, sliding walls -: multiple missile launchers pwnage
Hell +: awesome, portals, lazer eyes -: demon maws, demon maws, demon maws, lava balls, demon maws, demon maws
Rapture +: gravity orbs, open spaces, cool backgrounds, maggots, zombies, switches -: gravity orbs, excess of vanishing walls, flies
The End +: simple, aesthetically pleasing, pleasantly challenging -: way, way, way too long
Cotton Alley +: MUSIC, pink, sense of fulfillment upon completion -: like getting your head kicked in by steel toed boots over and over, every frustrating mechanic returns on steroids (demon maws, flies, waiting around, gravity orbs, tiny saws, really long levels)
Of course, some people may enjoy dying after 40 seconds of sustained gameplay repeatedly. Kudos to them.

Definitely The End to my opinion. All of its levels are based on pure technique (no vans, anti-gravity things, missiles, portals and all that stuff that i don't like much) and are not too short. Speedrun is a real pleasure on The End. Moreover, i love the level design of it. 6-1x is one of my favourite level of the game, if not my #1.
In second, i think i'd put Cotton Alley (and/or Hell).

inb4 redundancy?  Hell is probably my favorite world visually. Everything about it just seems unique compared to the other levels (Except possibly The End). Demon Maws on the other hand... they can really seem like overkill in some places, but once you know how to manipulate them properly, they really are more of a pushover. I like playing levels with Demon Maws, even Patience I enjoyed quite a bit!

Salt Factory for me.
It's really where the game came together for me and really showed it's greatness. Great fun/fair challenge, amazing music, and it's the first world where you're required and rewarded for refining your technique. And the artstyle was fantastic, it's probably my favorite because of how well Meat Boy's color contrasts w/ the visuals.
