I'm surprised with the amount of people who aren't saying the Dark World Dr. Fetus. The first section is so frustrating because of the fact you have to wait about 10 seconds every time you die for the saws to start movie, and the second part has some of the trickiest jumps I've ever seen in the game.
I found the difficulty to be a natural extention of the going through the light world version.  You only have to dodge two missiles before the saws start moving.  After that you don't have time to worry about Fetus.

I actually had more trouble in light world because I was running in high detail and it wouldn't sync right.  I only won because of a glitch where the saws pass through you and you can wait for fetus to catch up.

I think heīs referring to the time of waiting, not the difficulty of dodging the missiles.
Letīs say someone has problems navigating the first set of saw clocks, they have to wait 15+ seconds to try that part again. That grinds away your will to complete the level.

That is probably also the reason why Skyscraper is considered so hard, you have 1 difficult jump (The falling saws at the end of part 1), but itīs a long run to get there.

For me, the hardest level is still XOXO, despite the troubles I had with Skyscraper and The Guy 3 (yes, 3 Oo). I gladly went back to Skyscraper 2 times already, to improve my time, but I really dread playing XOXO again.

XOXO is the only level I have attempted a serious speedrun on, and I currently have the #2 in the world on 360, which is faster than any PC times and is very probably the legit #1 in the world as the 360 #1 is from a banned account.

From my experience i'd say it's possibly the level in the game with the biggest difference in percieved difficulty vs actual difficulty. The only hard bit is the top level, but once you get the top jumps into your muscle memory, you can do it without dying about 90% of the time, and that's while speedrunning it.

The trick is to walk the whole top section and not to time your jumps to jump the gaps, but to time them to hit the ceiling before the jump. That way you jump earlier and your body falls neatly onto the next ledge and misses the saws by a mile. Becomes extremely easy.

Hope this isn't considered a bump. Aside from the IWBTG levels (which are absolutely insane) I've got a few of my own.

4-3X Char: God... I played this at least one hundred times. No lie. I hate this level, hate it hate it hate it hate it hate it into infinity. But I'm trying to go for 100 Bandages (up to 81 now) and I was determined to get it. What gets me is that you have to be absolutely perfect. Total accuracy. The other hard levels at least let you jump around a bit and aren't instant death. But not this one. God almighty. The Devs could've slowed down the lava by maybe 2 seconds, would that be okay? Needless to say when I finally beat it (literally played almost two hours straight) I triumphantly yelled "F U" at the screen. True story.

5-5X Nix: Not that hard unless you want that Band-aid. The first part isn't too hard if you're Meatboy, sure you have to be accurate but you're given a little bit of leeway. But if you want that Band-aid it seems like the best overall character for that is Naija. Ogmo was too slow, couldn't get up there quickly enough. Pretty much all of the other characters are just too slow. Naija comes in handy for a few Magnet-based levels. But trying to get through this level, and get the Band-aid with Naija is torture. You have to be absolutely flawless here. Once you get the Band-aid you have to position your self back where you started, and if you overshoot it you get locked out one way or another, and if you're to the left of your position and hit the switch it seems like you don't have enough momentum to make it through. Then after all that, you have to figure out how to get around those giant saw blades with her pretty random teleport. Dear god.

5-19X Magog. Not too bad, I will call out the devs in that the homing saw blade being hidden under your timer is a cheap beginner's trap. But it isn't too hard until you get to the part where you have to wall-jump with disappearing-reappearing walls lined with Saws. I tried with Meatboy and it seemed like you once you got down some sort of pattern, you'd jump with enough force to send you flying into a Saw. So I had to lame it out with Jill, which isn't fun for the first half considering her slowness. I then proceeded to float as much as possible in that wall-jump death arena, my thumb is still killing me.

To me none of these levels are the hardest and not even close to it, But if you really have 81 band-aids and didnt collect the one on Char you fail

I did the Kid without too much of a hassle. Millennium, however, can die in a fire. I've tried to get through that stage for days.

Well, twice as many here find Kid2 harder. I thought Kid2 was tough, but less so than Dark Dr Fetus. After twenty-thirty minutes the first two elevators become easy, and then it's just a little practice nailing the timing on the third elevator, which in fairness is pretty much the same as the second one, except it's harder to start going down. I expect most people complete the fourth elevator on their first try.

Kid1, on the other hand, took me hours.

Naturally, those who had severe problems with Kid 1 never saw Kid 2. :-)
(Personally I found Kid 3 to be the hardest of the 3, but that might have been exhaustion)

I just went and got a couple of bandages, one of which was Millenium. Second time completing that level, quite surprised it's regarded amongst the hardest on this poll.

I'm sorry, but Millenium is nearly impossible for me. I've gotten the Kid and I now have four no-death runs, but this level kicks my ass repeatedly. I mean, getting through it normally seems bad enough (if the giant missiles don't kill me the small ones do), but to get it with the bandage you need to perform some truly ludicrous acrobatics while dodging missiles. I hope I'll get it eventually, but then... well, then I have to do the remaining three stages too.
