That's probably an unused alternative for either the "zombie" Meat Boys or a Hell alternative to the regular Meat Boy you play as.

Probably would have been an obstacle for the fight with Little Horn. It appears that burning Meat Boys are falling from the ceiling whenever he roars.

Probably the best guess. Gotta take a closer look at them.
Would be funny to have them as a "Punch the Keys" unlock to play with.

Yeah they are those flaming meat boys that fall. they originally use to charge at you like they do in ch5 but we removed that because the boss became way too hard.

But that doesn't rule them out as a sprite-swap for custom stages as the same enemies from The Rapture, right?
